These Terms of Use govern your use of www.modernholistichealth.com (the “Site”), operated by Modern Holistic Health, LLC. By using the Site, you agree to these terms. Services you purchase are also governed by our Park City Clinic Patient Policies or Nationwide Health Coaching Terms, as applicable, and our Privacy Policy.

Site content is for general education only. It is not medical advice, diagnosis, or treatment, and using the Site does not create a clinician–patient or coaching relationship. See our Scope of Services & Medical Disclaimer. In an emergency, call 911.

ELIGIBILITY AND ACCOUNTS
You must be 18 or older to book services or create an account on your own behalf. A parent or legal guardian must book clinic services for a patient or coaching client under 18. Booking, forms, and client accounts are provided through Practice Better, and you are responsible for keeping your login information confidential.

BOOKINGS AND PURCHASES
Prices and service availability may change without notice. Clinic services are provided only in person at 1910 Prospector Ave., Suite 204, Park City, Utah 84068. We may cancel any booking or order we cannot fulfill and will refund amounts paid for it.

INTELLECTUAL PROPERTY
The Site’s content, including text, graphics, videos, program materials, and logos, belongs to Modern Holistic Health, LLC or its licensors. You may view and print content for personal, non-commercial use. You may not copy, distribute, or sell it without written permission.

ACCEPTABLE USE
You agree not to misuse the Site, including by attempting unauthorized access, interfering with its operation, submitting false information, or posting unlawful or abusive content.

REVIEWS AND TESTIMONIALS
Reviews and testimonials on the Site come from our public review platforms (including Boast and Google) or are shared with the client’s written permission. They reflect individual experiences; results vary, and they are not a guarantee of similar results. We do not pay for reviews or selectively suppress negative reviews.

THIRD-PARTY LINKS AND SERVICES
The Site may link to or use third-party services, such as Practice Better, Zoom, payment processors, laboratories, and review platforms. We are not responsible for their content or practices; their own terms and privacy policies apply.

DISCLAIMERS
The Site is provided “as is” without warranties of any kind, to the extent permitted by law. We do not warrant that the Site will be uninterrupted or error-free.

LIMITATION OF LIABILITY
To the fullest extent permitted by law, Modern Holistic Health, LLC is not liable for indirect, incidental, or consequential damages arising from your use of the Site. Nothing in these terms limits liability that cannot be limited under applicable law, including liability for professional negligence in clinical care.

GOVERNING LAW
These terms are governed by Utah law. Disputes will be resolved in the state or federal courts located in Summit County, Utah, unless the law of your state requires otherwise.
