Modern Holistic Health, LLC (“Modern Holistic Health,” “we,” “us”) offers two distinct types of services. They differ in who provides them, where they are available, and what they include. Please read this page to understand which service you are receiving.

PARK CITY CLINIC (IN-PERSON ONLY)

Our integrated clinic is located at 1910 Prospector Ave., Suite 204, Park City, Utah 84068. Clinical services are provided only in person at this location and only to patients who are physically present in Utah at the time of service.

Clinic services are provided by licensed clinicians, which may include physicians (MD/DO/DC), nurse practitioners, chiropractors, and other licensed professionals, each acting within the scope of their Utah license. Not every service is appropriate for every patient, and not every clinician provides every service.

CLINIC SERVICES

• Functional/holistic healthcare
• Integrative medical evaluation and laboratory testing
• Bioidentical hormone therapy, including compounded hormones
• Peptide therapy
• IV and nutrient therapy
• Platelet-rich plasma (PRP)
• Regenerative and biologic therapies, including stem cell and exosome therapies

IMPORTANT INFORMATION ABOUT CLINIC THERAPIES

Some therapies we offer are off-label, compounded, investigational, or not approved by the U.S. Food and Drug Administration (FDA) for the use being discussed. Compounded medications are not FDA-approved. Stem cell, exosome, and other cellular or biologic products are generally not FDA-approved to treat, cure, or prevent any disease or condition. These therapies are offered only where permitted by law, after an individual evaluation, and after a discussion of the potential benefits, risks, alternatives, and costs. No therapy is guaranteed to work.

Clinic patients receive an Informed Consent and Treatment Agreement and any service-specific consents before care begins.

NATIONWIDE VIRTUAL HEALTH COACHING

Our health coaching program is delivered virtually by video, phone, and secure messaging to clients throughout the United States, except where not permitted by law. Health coaching is educational and supportive. Health coaches do not diagnose, treat, cure, or prevent any disease; do not prescribe or adjust medications; and do not provide medical, psychological, or other licensed clinical care.

Some of our coaches hold professional licenses or credentials. When coaching, they act only in a coaching capacity, and coaching does not create a physician–patient or other clinician–patient relationship. Coaching is not a substitute for care from your own physician or licensed healthcare provider. Please consult your healthcare provider before making changes to medications, diet, supplements, or exercise.

Clinical services from our Park City clinic are not available by telehealth to people located outside Utah.

EMERGENCIES

Neither the clinic nor the coaching program provides emergency services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. If you are in emotional crisis or thinking about suicide, call or text 988 (Suicide & Crisis Lifeline).
