Park City Clinic Patient Policy
Park City Clinic Patient Policy
Effective October 1, 2026
Applies to in-person services at our clinic at 1910 Prospector Ave., Suite 204, Park City, Utah 84068.
These policies help us provide safe, well-prepared care and keep appointments available for all patients. Clinic patients also sign an Informed Consent and Treatment Agreement at intake.
INTAKE FORMS AND RECORDS
Please complete all intake forms, health history, medication and supplement lists, and consents through our Practice Better patient portal at least 72 hours before your first visit, and any newly requested forms at least 72 hours before later visits.
If forms are not complete in time, we may reschedule your appointment so your clinician can review your information safely. A reschedule for incomplete forms within 72 hours of the visit is treated as a late cancellation.
Please bring photo ID and any recent lab results or records from other providers.
PATIENTS UNDER 18
We see patients under 18 with the consent of a parent or legal guardian, who must complete intake forms and consents and accompany the patient to visits unless otherwise permitted by law. The parent or guardian is responsible for payment.
Scheduling, cancellation, and no-shows
Please cancel or reschedule at least 72 hours before your appointment by calling 888-260-3377, emailing admin@modernholistichealth.com, or using the patient portal.
Cancellations or reschedules with less than 72 hours’ notice may be charged a late cancellation fee of [FEE — to be determined].
Missed appointments without notice (“no-shows”) are charged [FEE — to be determined].
Repeated late cancellations or no-shows may result in a prepayment requirement or discharge from the practice.
We understand that emergencies happen and may waive fees at our discretion.
PREPARED AND ORDERED PRODUCTS
Some treatments require us to order, prepare, or reserve products specifically for you, including IV therapy, PRP, stem cell and exosome products, and compounded medications. Payments for these products are non-refundable if you cancel, and also if you reschedule to a date after the product expires or can no longer be used.
LATE ARRIVALS
If you arrive late, your visit may be shortened or rescheduled so we can stay on time for other patients. Some procedures cannot be safely started late and may need to be rescheduled. Full fees apply for shortened visits.
PAYMENT
- Modern Holistic Health is a self-pay practice. We do not accept or bill health insurance, Medicare, or Medicaid. On request, we can provide an itemized receipt that you may submit to your insurer; reimbursement is not guaranteed.
- Payment is due at the time of service unless you have purchased a package or membership.
- By keeping a card on file, you authorize us to charge it for services received, late cancellation and no-show fees, and balances due.
- Good Faith Estimate: Because you are not using insurance, you have the right to receive a Good Faith Estimate of expected charges before your scheduled services, under the federal No Surprises Act. Ask our front desk or visit www.cms.gov/nosurprises.
PACKAGES AND MEMBERSHIPS
- Packages and memberships expire 12 months from the date of purchase, unless a different term is stated in writing in the specific package or program terms.
- Packages and memberships are non-transferable.
- There are no refunds for early cancellation of packages or memberships.
REFUNDS
- Services already provided are not refundable.
- Where a refund is due (for example, if we cancel a service we cannot provide), it is issued to the original payment method within 10 business days unless the original payment method is not available.
MEDICATIONS, SUPPLEMENTS, AND PRODUCTS
Please tell us about all prescriptions, supplements, and changes in your health. Products purchased in the clinic cannot be returned once opened, for safety reasons. Compounded medications are prepared for you individually and cannot be returned.
COMMUNICATION
For scheduling, billing, and non-urgent questions, contact us at 888-260-3377, admin@modernholistichealth.com, or through the patient portal. We respond Monday through Friday, 9:00 a.m. to 5:30 p.m. Mountain Time, and do not monitor messages outside business hours. Do not use email, text, or the portal for urgent medical concerns.
EMERGENCIES
The clinic does not provide emergency care. For a medical emergency, call 911 or go to the nearest emergency room. If you have a serious reaction after a treatment, seek emergency care first, then notify us.
RESPECTFUL ENVIRONMENT
We may decline or end care for anyone who is abusive or threatening to staff or other patients, repeatedly disregards these policies, or provides false information. When ending a care relationship, we will give reasonable notice and help with transfer of records where appropriate.
YOUR RECORDS AND PRIVACY
You may request a copy of your records at any time. See our Privacy Policy for how we protect your information and your rights.
